Possible Allergy: You may not be able to digest certain foods. Try keeping a food diary and see if a pattern or certain food causes problems, if so, avoid. Malabsorption.: This sounds like it could be related to malabsorption, perhaps related to the pancreas or biliary system in general. You need more of a work-up than you've had thus far. Fiber, hot baths: "rectal injury" from a large hard stool is most often an anal fissure that will heal in a few weeks with soft stools. Hot baths will help the muscles relax and reduce discomfort. Acute or chronic?: I am very concerned if your constipation is relatively acute, ;/or associated with fever, nausea, vomiting, bleeding. Constipation (stooling less than every 4 days) that is chronic raises concern for partial obstruction, ileus, ischemic disease, tumors, dysmotilities, "cathartic colon", rectal dysfunction.
