Orthopaedics eval: 31 year old gentleman with bilateral burning posterior knee pain, radiating pain from thigh to calf. You need a physical exam and evaluation from a specialist in orthopaedic surgery. Your physical exam will include a straight leg raising test to evaluate radiculopathy, pain along a spinal nerve from your spine which can radiate to the lower extremities. Imaging tests and other maneuvers will help.

Not Avelox: Medication problems do not choose just to affect one area, or one extremity. This problem could be hip or spine (especially the 4th spinal nerve). You should see your primary care 1st. Rj.

Nerve prob: Typically burning sensation is associated with a nerve irritation. You may have a pinched nerve somewhere in your back for ex. You should see your doctor.

Need more info: NEED PHYSICAL EXAM & DATA When did it start? What brought it on? What makes it better? worse? R U otherwise healthy? Did you have any recent accidents, or injuries? Are you physically active? Describe your day. Tell me how this problem interferes with your daily activities. Does this burning keep you from sleeping? Do you take any medicines, prescription or otherwise? Herbal? Street? Diet?

Yes: It sounds like you have some irritation of your nerves of the back which may be a radiculopathy (a specific nerve root) or sciatica (several nerve roots that join together into the sciatic nerve). Your physician can tell by your history and physical which nerve(s) are likely to be involved. Based on that he may recommend an MRI or physical therapy or medications.

Potential nerve: This could be a problem of the nerves that subserve that area. Please see your physician who can obtain a thorough history and perform a physical exam to determine if you need additional test like imaging and/or electrodiagnostics.

See neurologist or..: Your described characters of symptoms deserves a timely professional attention from neurologist or urologist ASAP. So, collect the onset, degree, quality, & quantity of the symptoms with their detailed sequence of events, & bring those to Doc for review, analysis, physicals, & possible test so to deduce possible Dx for proposing reasonable Rx. Clinically, one needs to think neuralgia/radiculopathy

Meralgia Paresthetic: I actually didn't have room in the SUMMARY above to add the last letter of "a" to the name of what I wanted to write. So, Meralgia Paresthetica is a condition where the femoral nerve is trapped at the level of where it enters the thigh. Numbness and burning sensations can arise. Often it is on both sides but can be only on one. See your doctor and if appropriate a neurologist.

Hip arthritis or: sciatica, low back lumbar disc problem, but if pain located over hip probably hip but if radiates form low back to outer thigh could be pressure on sciatic nerve, if persist for weeks need to get xray and mri of hip and lumbar, if worse or loose bowel and bladder control , emergency called quada iquana syndrome[severe pressure on nerve]

Neuralgia: Nerve pain can take many forms, with burning or stabbing being common. A pain in the anterior and lateral thigh may be due to meralgia paresthetica, which simply is a painful syndrome of the nerve serving that area. Other possibilities are a pinched nerve in the back. Rarely, diseases such as lyme or cancer, or trauma can affect these nerves.

Contact Dermatitis?: It is very difficult to tell what this might be without seeing it. I have seen people who are allergic to nickel metal and who have contracted this itchy skin condition by sitting on metal or cast-iron benches. One lady worked in an outdoor furniture shop recurrently had this dermatitis. In short, it's very essential to see a dermatologist to have proper assessment of this problem.

Entrapment?: If pain is truly random, i.e., not related to position or activity, you might have an entrapment neuropathy of a branch of medial femoral cutaneous nerve--not a common condition. Knee problems often cause pain in that area. Are you having joint stiffness, clicking, locking, swelling, heat?

See below: If the pain is on the outside of the thigh, ou may be experiencing a condition called meralgia peristhetica which is a pinched nerve near the hip. Stretching helps and should be the first thing attempted.
