Chondromalacia: Yes, chondromalacia with cyst means there is some cartilage damage present. Therapy can be helpful in strengthening the quads and helping the kneecap track in its groove better. Therapy doesn't make the cyst or chondromalacia go away but can be helpful for the pain.

Possibly see ortho: Difficult problem. Especially if your symptoms correlate with your MRI findings. If your pain is behind the knee cap then there is good correlation. Initially nonoperative intervention such as therapy, use of anti inflammatories and patellar brace may be helpful. The possibility of arthroscopic evaluation if all fails may be necessary but still a difficult problem to fix. Supplements controversial.

At least 6 months of nonoperative treatment is attempted before offering arthroscopic surgery.

Several: Commonly, stretching quads, building quads, avoiding deep knee bending exercises and stairs. Some patella braces and taping can help in some situations, injections with hyaluronic acid liquids, sometimes arthroscopy, sometimes cartilage paste grafts, sometimes patella realignment surgeries. You need a good exam and knee doctor to tell you what you need.

Be patient: Exercize wizely. Wear a patella tracking brace for the activities that cause the pain. Good nutrition. Lose weight if you have too much. Strengthe the inner quad muscles preferentially.

I've chondromalacia patella and have been strengthening my vmos to resolve my patellofemoral pain syndrome. How long before the chondromalacia is gone?

Months: Can take several months. Need to add a core strengthening program and stretching too. If its not responding by then, go see your orthopedist (again?).

Gentle exercises: Chondromalacia is a softening of the cartilage between the patella (knee cap) and underlying joint. The best early treatment is to gently strengthen the quads (thigh muscle). Step up with your painful leg onto a 4" block/book and down. Repeat as many reps as comfortable and work up to doing 200/session. Then do it on a stair, which is typically 6". This is to avoid too much knee bend/strain.
