Fissure, hemorrhoid: At your age this sounds like an anal fissure which is a tear of the lining of the outlet. Hemorrhoids rarely cause real pain unless one gets a blood clot which would be a hard lump at outlet and hurt all the time not just with BM. Constipation. Abdominal distension. 2x bm/mo only with laxative.Extremely painful with a lot of red blood in toilet. Anus feels pushed out (?).
Bad constipation: You may have hemorrhoids from all the straining. You need a better bowel regimen of medicine to have more frequent bowel movements. Stool softeners (like colace) do little to help. Try mirilax (you can take daily and very safe but take few days to work). You should also try an enema daily.
