Cyclical: The cyclical pain that goes away is a good sign really. Is the area affected up toward the armpit? If so, then that could be indicator that the breast tissue is getting tender from the hormonal input to it and then resides as the hormones change. A Shoulder; A Neck or Upper T. Spine, skin or deeper tissues causes: from any nerve, bone, lung, lining of chest cavity. Some skin causes would include an Infection of sweat glands, or around the hair there.
If its occurring randomly, take some Aleve or Tylenol (acetaminophen) for a week or so.
If it persists or is getting worse, see your PCP.

May be serious: How long have you had these symptoms and how severe are they? If of recent onset and pain is 10 out 10, it is safest to go to the er or even to call 911. Herpes? STD?: HIV is not a likely cause. Herpes could cause multiple oral ulcers, usually not just one. Painful urination could indicate an STD like gonorrhea, chlamydia, or herpes, especially if there is also discharge from the penis. (But I haven't a clue how to interpret armpit pain when urinating.)
