Surgery not required: Anal fissures can usually be treated conservatively. In fact, a lengthy attempt at conservative therapy is usually best in my opinion. Yes: Pouchitis? I assume your colostomy gets inflamed. Begin with diet and start a low fiber diet. If no better after 1 or 2 weeks try bulking agents ( fibercon or equivelent). It's counter intuitive so first try no fiber then trying more fiber to see which helps. If still a problem consider small amounts of Imodium (loperamide) regularly.
