Habit?: Some boys will get an irritation in that area from the heat and being sweaty. Since it's causing symptoms I would apply 1 % hydrocortisone cream and some baby powder
Use both twice a day. Also if he still has accidents at night make sure you wash him in am, Sometimes boys of yhis age just get into the habit of touching and scratching ...Read more

Workup/Mgt: I agree that a food allergy panel called a CAP test is indicated. If the rash occurred particularly within 4 hours of eating the tomatoes, an allergic reaction is most likely. The itchiness as well indicates an allergic reaction. Avoiding the food, and using an antihistamine by mouth is part of the management. I would also suggest a discussion about pediatric epi pen with your physician. ...Read more

Rashes: Are hard to diagnose without being seen. A rash in the inner elbow area could be eczema. You can try coconut oil or aveeno (oatmeal) lotion and see if tat helps. Possibilities: It sounds like this could be ringworm- usually this happens in perineum or areas where skin rubs against skin, and can be more diffuse. Another thought is impetigo, another rash that can spread among people in close contact. Best for your md to see. ...Read more

Fifths: Fifths disease is a common, relatively harmless viral infection - most often seen in kids and characterized by red cheeks followed by a lacy rash on the arms, legs and body. Some people will also have fever for a day or two. It typically resolves on its own without treatment. The only time we really worry about this virus is if it infects someone who is pregnant or someone with a chronic blood disease such as sickle cell anemia, thalessemia, or spherocytosis. The virus can stop the bone marrow from producing red blood cells for a short period of time. Interesting: Red raised sounds like hives...And why monthly? Do not know. The low grade fever may be from the rash itself.... Do a diet chart and see if there is something that stands out...You will have to do it for a month. Consider allergy testing and seeing a chinese medicine practitioner...And immunologist as well.
