See your doctor: With anal fissures, it is likely you have seepage of stool or liquid from the rectum that is irritation the skin, It can be very hard to keep the area clean and dry when this starts to occur. Have you seen anyone about surgery for the fissure. I would recommend you do.

Frequent pad changes: It could be because you have sensitive skin and is having some reaction to the sanitary pads. You need to change your pads every 2-3 hours, and even more frequently if necessary.

Itchy vulva: My advice is to avoid tight clothing, wear white cotton panties and obtain a cream like mycolog which contains an antifungal, antiyeast medication combined with a cortisone type medication which relieves irritation and itching. Hopefully you won't have to visit a gynecologist.

See PCP: See your primary care doctor first. Some PCP's will not do pelvic exams so check with yours to see if you should see a GYN doc instead. If you normally see a GYN for pelvic exams go there. Health departments can sometimes evaluate and treat for pelvic infections/problems.

Very itchy rash under arms and breast, behind knees and ears, inside elbows and in vaginal and anal area. Skin biopsy show eosinophils. What does this mean? Could this be a food sensitivity?

Eosinophilic rashes: A variety of diseases can cause diffused skin rashes with eosinophilic infiltration including those mentioned in previous answers. However it seems like it affects both skin and mucus membrane and it is very diffused. Hypereosinophilic syndrome is a concern. Either an allergist, dermatologist, or hematologist can help you in this case.

Intertrigo: The rash could be intertrigo or a cellulitis. If it is hot to touch, see a physician as soon as possible to make sure it is not a cellulitis which would need oral antibiotics quickly to treat. Intertrigo is a rash that occurs in folds of skin and can be treated with topical creams usually.

Yeast infection?: The main cause of symptoms like this is vaginal yeast infection. Best to see a doctor and follow her advice for treatment. Or you might try an over-the-counter treatment (look for a product with active incredient ending in "azole", e.g. miconazole, clotrimazole, etc. But for sure see a doctor if symptoms don't clear up in a few days.
