Pregnancy: Yes, pregnancy can be uncomfortable at times, especially as you get further along. I am guessing that you have been seeing your OB regularly and all is well? Get their "okay" to try a prenatal yoga class, or get a prenatal massage to help with the low back pain.

Multiple causes: Trauma to the vaginal area from rough sex, manual manipulation, infections, cysts, polyps, tumors, irritation of nerves serving vaginal territory, muscle spasms, hormonal fluctuations as in menopause, and many other causes to VULVODYNIA are possible. See your doctor if it persists. Most of the time these pains will resolve and it'll be unclear as why it came about in the first place.

Any severe pain: Even if it is intermittent should be evaluated by a physician. In your case you will need imaging studies too. Depending upon where you live it may be faster to call your doc and see what they suggest. If that's not possible then consider a visit to an urgent care or an ER. B4 you go to urgent care call to see if they can test urine, blood and do X-rays on site. ERs and UC will be busy, call first

Could get checked: If you are on Depo then you do not have to worry about pregnancy. Irregular bleeding happens on Depo sometimes. You can schedule a visit for an exam to have the vaginal pain evaluated and you can ask for STD testing.

Pain: It's probably round ligament pain and certainly you can try taking some tylenol (acetaminophen) or acetominophen to see if it relieves it. However, it never hurts to call your OB physician or midwife and ask if they want you monitored for labor and checked for infection.

See you OB: You should call and see your OB ASAP. Twins increased your risk of preterm labor. 18 weeks is a bit early, but, if nothing more than for your peace of mind, I would get checked.

Call your OB now: Call your OB now. You could be in labor or it could be Braxton Hicks contractions, (false labor). At 34 weeks, you do not need to worry about about a premature baby, your baby's lungs have sufficiently developed. Congratulations and good luck.
