This is serious: A nephrologist or pathologist needs to look at the urine to confirm the presence of red cell casts. If they are indeed present, you have glomerulonephritis of some sort. Since you state that you have systemic lupus, this is the obvious first consideration, and the presumptive (and likely) diagnosis is lupus glomerulonephritis. Obstructed?: Obstruction of the upper part of the ureter or within the kidney could do this. e.g. from a kidney stone or papillary necrosis. That is, the infected area is not draining, and so no bacteria or white blood cells in the urine. If so, it's quite serious. Or maybe infection with a bacteria not detected on routine urine culture, but that's very rare. Keep working closely with your doctors. ...Read more

UTI: White blood cells in the urine indicate a urinary tract infection (uti), usually a bladder infection (cystitis) but sometimes a kidney infection (pyelonephritis). In the face of that finding, i don't see how you can "be sure" you don't have a UTI or kidney infection, i think you actually do. See a doctor and get further evaluated and treated. ...Read more

Urinalysis: The ketones may have been because you were fasting when collecting the urine, or perhaps you are diabetic and blood sugar not in good control. The leukocytes were probably from vaginal secretions unless you had a clean assisted midstream catch urine. You do not say if you have any symptoms. Lab values without a clinical context are not very valuable. ...Read more

It's high: If by '30' you mean 30, 000, that's extremely high. If you have a kidney infection, are sweating, and have a WBC of 30, 000, you should be seen by a doctor right away. ...Read more

No problem but..: all ok except blood in urine unless you are on menses, should have repeat in 2-4 weeks , could be kidney stones or uti , especially if symptoms of burning on urination . unless symptoms no problem to have some bacteria ...Read more
