Depends: on how long it has been present, if it is changing (enlarging or changing color) or if it appeared suddenly or after local irritation. It's not uncommon to have tiny purple spots due to bruising from breastfeeding or using a breast pump. See your doctor: If the spots are away from the nipple it could be anything like a bruise or bite or insect bite. If they are close to the nipple they can be inflamed montgomery glands (they lubricate the nipple). Go see your doc if it does not spontaneously go away. Do you have nipple discharge?
