Punctured wound: Development of an infection is greatest concern and will depend on how deep the nail got. If shallow, rate of infection should be almost minimal. If deep, surface cleaning make not be enough to prevent bacterial infection. Children typically receive their first tetanus shot at 2 months, the second at 4 months, and the third at 6 months, the fourth between 15 and 18 months, and the fifth between 4 and 6 years of age. Then, the next dose is between ages 11 and 12 years old, and then up to every 10 years afterwards.
I would clean the wound with soap and water, and then take your child to your regular doctor. Probably your child is up to date on his tetanus shots - but your child's doctor would know for sure (although he wouldn't have received the 5th dose of the primary series). Your doctor can evaluate the wound and the piece he fell on - in general, if the metal is not rusty or "dirty" (i.e. Was lying in a cow pasture) it should be okay. However, seeing your doctor is a good idea to see also if the scrapes and the wound require stitches, and possibly topical or oral antibiotics.

Depends: Today it is common for OBGYNs to recommend the TdaP vaccine during pregnancy, but people remember it more for the pertussis part. No: Your prior booster in 09 will have you covered. If you had dragged your open wound through a lot of soil and debris, and left it untended over a long period, it might be a benefit to some. Tetanus: This is uncommon in the USA because of immunizations. There are less than 70 cases a year. Since boosters are recommended every 10 years you should be fine, but it would not hurt if you went and got the booster now. You don't say where the puncture wound was.
