R/O IBS: IBS is a diagnosis of exclusion. IUC/CROHNS DZ/IBD is inflammation usually DX by BX IBS SX can be managed .You need a full exam and blood work. Especially screen for H pylori . Could be hernia.: A hernia could exist without a discernible bulge. The other symptoms are consistent with a hernia. Could also be kidney stone, an issue with the sigmoid colon, pelvic lymph node enlargement...in short, you need some testing. Depends: I would like to know where the pain starts. If it originates at the buttock it is possible that the problem could be coming from the back despite not having back pain. My advice would be to get an EMG to rule out a radiculopathy, consider imaging of L-spine and Right Hip, and based on these results decide further tx.
