Psa: New aua guidelines indicate psa above 2.5 may indicate a need for a biopsy. In a 60 yo with a family history and a psa of 3.1 I would advise bx if pt has >10 year life expectany, if no family history I would be less adamant...But I would offer bx or repeat psa in 3-6 months.
