What is the best medicine for OCD. We have tried Zoloft, lexapro, wellbutrin, prozac. Side effects bad or did not work. Does Pristiq (desvenlafaxine) work for OCD?

Agree w/Dr. Buggie: I agree with dr. Buggie about the need for psychiatric evaluation and ongoing treatment. Wellbutrin does not help with anxiety. Paxil (paroxetine) is a reasonable choice, but whether it's right for you I cannot say. You're also on Adderall xr, which can exacerbate anxiety in some. A: A combination of cognitive behavioral therapy and anti depressants is an effective treatment strategy. There are other anti depressants with a different mechanism of action that could be tried- eg: tricyclic anti depressants (nortriptyline), mirtazapine, MAO inhibitors. The treatment could also be augmented with medications in the class of Abilify (aripiprazole), seroquel etc. If nothing else works, electroconvulsive therapy is a safe and effective treatment option. Depends: Sometimes starting on an antidepressant or increasing the dose can increase anxiety. If it is not tolerable, call your doctor. Slow down on the dose increases. Tolerate your treatment at every step! If anxiety is gets higher despite treatment with no dose change, call your doctor to address all forms of treatment. Experiment: A 5mg dose of Lexapro (escitalopram) is a starting dose. If you have given it at least 1-2 weeks, call your fp and let them know that you may need more milligrams. A 10mg dose would be a standard dose, with a possible 20mg maximum next month. If you anxiety is truly "major, " then you might also discuss add-on therapy for break-through panic symptoms. Steady state depends on half-life (among other things), which is roughly 30 hours. This means it takes 30 hours for your body to metabolize half of the Lexapro (escitalopram) dose taken.
