I have dull burning in abdomen with indigestion and tenderness in center of breastbone. Probiotics help. also taking nexium (esomeprazole) 20mg and mylanta. ?

There is no one best: medication for GERD. the best med will be the one that you & your doc find helps you the best, with the fewest side effects. ranitidine is an H2 blocker, as is pepcid; while nexium (esomeprazole) is a "PPI" or proton pump inhibitor. it seems unlikely, that if a PPI + an H2 blocker is not fully effective now, that an H2 blocker alone (ranitidine) would be effective. review ur sxs w/ ur MD to find the best tx 4 u! ...Read moreSee 2 more doctor answers
Reflux: You may have gastroesophageal reflux disease. Have you noticed it gets worse after eating especially spicy foods or drinking coffee? What about laying down after eating. Several things: first, results from endo including H Pylori presence/absence would be helpful. Bloating below stomach isn't stomach. Then, omeprazole is weak in two regards 1) it is the weakest of the proton pump inhibitors and 2) proton pump inhibitors work for about 20 hrs not 24, taking a PPI twice a day increases its effectiveness on GERD but not bloating. ...Read more

?endoscopy: Gerd can burning in throat and chest discomfort as well as causing food to get caught in esophagus. Costochondritis is inflammation of junction of ribs with cartilage in front of chest along the side of sternum. Usually tenderness to touch and pain with movement but not burning in throat. Gastritis & ulcer: If you're still suffering from burning pain in stomach despite taking Protonix (pantoprazole) every day, ask your prescribing doc for another option like omeprazole or lansoprazole. Think of Chevy vs Dodge vs Ford (or Honda vs Nissan vs Toyota). All make reasonable cars but one might have better product for your specific needs. Same concept applies to PPIs. You need treatment: It sounds like ur symptoms still aren't controlled -so I hope your doc could keep 'working at it' with u. Somethings that have helped my patients (& don't require the doc) are adjunctive therapies. You may want to try dark chocolate with mint (Lindt has a good one) -Chocolate was used by the Aztecs for stomach trouble, and mint helps the tummy. Prilosec: Prilosec and similar medications tend to be better at preventing acid reflux than in treating it once it has already started. If you have only occasional heartburn, often the best treatment is good old-fashioned chewable antacids, such as Mylanta or Tums (calcium carbonate). Milk does temporarily neutralize acid, but as any food, it will stimulate more acid production in the stomach as well. In a pinch, it will provide some relief for a short period of time.
