Zofran (ondansetron) doesn't work: for a lot of people, myself included (it may be genetic). Tell your ENT you're still having difficulty, and see if perhaps s/he would recommend Meclizine. See my checklist on dealing with nausea. ...Read more

Likely central: Hi Joe, dizziness with visual acuity changes and hyperacusis may point to a central problem rather than a problem with your inner ear. One of the more common conditions associated with this constellation of symptoms is migraine variant. I'd consider obtaining vestibular testing and if negative, a neurologic evaluation. ...Read more

Comments: Downbeat nystagmus is a sign of cerebellar/ brainstem connection pathology, and may or may not relate to an effect of the Chiari malformation. This needs further evaluation by a neuro-ophthalmologist, and perhaps a neurosurgeon who has prior EXPERTISE in Chiari, as can be associated with other problems. ...Read more

Get second opinion: Roaring in the ear plus dizziness is very suggestive for menieres. There is no definitive test for menieres. The diagnosis is based on detailed history and testing. Would recommend you get a second opinion with an otologist. This is an otolaryngologist who sub-specializes in ear disorders. Hard to guess: A urine pregnancy test, done correctly, gives a reliable result 3 weeks after sex (3 weeks after when conception might have occurred), 1 week after missing a period, or 1 week after starting a "weird" period. Maybe: Dizziness could certainly be related to rapid heart beat. Can't relate the eye symptoms though. I see you've had an echo, EKG, and chest CT - ask your doctor for an event monitor: 24 hour, 48 hour, or 30 days - depending on how often you get your symptoms to try to catch the rhythm when you're having symptoms. ...Read more
