Lupus most likely: You are describing a complex of symptoms and findings consistent with lupus. The positive ana, rash and joint pain gives you a 75% likelihood fo having lupus. Yes: These symptoms sound like central (brain) disorder. The EEG may give important information. The fact that prednisone helped may indicate an inflammatory disorder. See a neurologist. Hope yes: If you have serious symptoms, you would feel better to see an ENT who does the Epley maneuver. Then you could at home do a series of Brandt Daroff exercises. You have not been well served with the prednisone. Both maneuvers are well described on YouTube. I show my patients the Brandt Daroff exercises often. Palpitations: No. Experiencing palpitations is not normal. About 1-2% of patients taking stroid therapy reported palpitations as side effect. Some times prolonged steroid therapy could cause cortisol deficiency, which could lead to hypotension; tachycardia (rapid heart beat) could be a reflex response. Must consider infect: With the long term steroids even with a low temp we must consider infection. Sometimes this can occur when you get a new Prednisone prescription and the new pills are either ineffective or are not bio equivalent to the previous Prednisone pills you need to see your rheumatologist for an evaluation

Rheumatologist: You must be seeing a physician for this condition. Have them consider consultation with a neurologist. More info: Do the palpitations awaken you or do you awaken with a startle which causes the heart to race? How fast is your pulse when you note the palpitations? How long do they last? Do you also get palpitations on other occasions. A pulse rate of 120 to 150 has more significance than one of 90, for example. A startle reaction may last a minute or 2 as opposed to significant tachycardia. Heart monitor helps.

Easy things to check first and foremost would be hydration, good nutritional habits, correction of anemia if present, reducing/eliminating smoking and excessive drinking.
