How is an anal fissure surgery done? Is it successful? Will the pain go after surgery? Will the fissure recur? Want to be pain free asap.
More precise: Lateral internal sphincterotomies treat chronic anal fissures. Traditionally the internal sphincter muscle was divided all the way up to the dentate line. This sometimes resulted in incontinence. Tailored sphincerotomy divides the muscle only as high as the fissure goes. Some surgeons modify this if they measure for anal stenosis. Studies show less risk of incontinence with tailored procedures. ...Read more

Meds, possibly Botox: There are topical medications (in cream form) that can be used to help fissures heal; diltiazem is prob most commonly used. For patients who fail to heal with this, injection of Botox to temporarily paralyze the internal sphincter can produce healing. I try to avoid sphincterotomy in women due to the higher risk of continence impairment (higher than in men). ...Read more

Is a C-section considered medically necessary with an unhealed anal fissure? I had surgery year before for fissure, and it never healed.
Depends: Without seeing your entire medical history, as well as the reason for the fissure and why it didn't heal, it's a little difficult to say whether cesarean indicated or not. If you have an area close to the perineum that has not healed, I don't think this is Cesarean is a bad idea at all. That's a delicate area and healing from a cesarean is not bad . ...Read more

Chronic fissure: Initial chronic fissure treatment includes correcting any constipation or diarrhea along with medicated creams (nitroglycerin ointment and/or diltiazem).
