Anal fissure surgery, tailored sphincterotomy, can you tell me more?

More precise: Lateral internal sphincterotomies treat chronic anal fissures. Traditionally the internal sphincter muscle was divided all the way up to the dentate line. This sometimes resulted in incontinence. Tailored sphincerotomy divides the muscle only as high as the fissure goes. Some surgeons modify this if they measure for anal stenosis. Studies show less risk of incontinence with tailored procedures. ...Read more

Meds, possibly Botox: There are topical medications (in cream form) that can be used to help fissures heal; diltiazem is prob most commonly used. For patients who fail to heal with this, injection of Botox to temporarily paralyze the internal sphincter can produce healing. I try to avoid sphincterotomy in women due to the higher risk of continence impairment (higher than in men). ...Read more

Is a C-section considered medically necessary with an unhealed anal fissure? I had surgery year before for fissure, and it never healed.

Depends: Without seeing your entire medical history, as well as the reason for the fissure and why it didn't heal, it's a little difficult to say whether cesarean indicated or not. If you have an area close to the perineum that has not healed, I don't think this is Cesarean is a bad idea at all. That's a delicate area and healing from a cesarean is not bad . ...Read more

Chronic fissure: Initial chronic fissure treatment includes correcting any constipation or diarrhea along with medicated creams (nitroglycerin ointment and/or diltiazem). The meds are directed at physiologic relaxation of the internal sphincter. Should the fissure does not heal. despite these measures, then sphincterotomy is the next step and should allow healinge. Would recommend seeing a colon & rectal surgeon. ...Read more

To avoid infection in the ass, what are precautions for someone with an anal fissure when using public places such as pools, bathrooms, gym showers, and toilets?

Gym anal infections: It is almost unheard of picking up any type of anal infection in a patient with a fissure from coming in contact with any surface.
However as a precaution, I do recommend all patients place a towel when coming in contact with a warm moist surface that is shared such as a steam room, sauna etc. Although rare, it is feasible for anyone to get anal condylomata (HPV) in that manner with or fissure ...Read more

I was checked out by my doctor and she said I have an anal fissure. My question is, do anal fissures bleed a lot? And do they cause blood clots?

Depends: There are a lot of qualitative terms in your question.

Anal fissures normall cause bleeding that you see on the toilet paper or toilet bowl. A small drop of blood in a large bowl of water can seem like a lot of blood.

Depends on how you are rating what a lot of blood is, when and where you are seeing it and other symptoms.

Blood clots are not typical for a fissure, but possible. ...Read more
