Plavix (clopidogrel): Plavix (clopidogrel) is used for specific purpose in specific doses. Some people do require more Plavix (clopidogrel) dose than others. Too much Plavix (clopidogrel) can be associated with too much bleeding.

Used in the treatment of heart attacks and strokes to decrease recurrent events.

Many reasons: Its used with Aspirin when pts have coronary stents placed, peripheral arterial disease especially if stents were placed, carotid dz again especially with stenting.Pts with vascular dz and Aspirin allergies. Just to name a few.

Similar: Plavix (clopidogrel) and clazol both inhibit platelet aggregation. Plavix (clopidogrel) is associated with a slighly higher risk of lowering the platelet count than clazol. However, clazol is usually contraindicated in patients with class 3 or 4 heart failure.

Celexa (citalopram): The usual starting dose is 20mg once daily. I feel that collaboration between the cardiologist and the psychiatrist is advisable in this case in view of your mom's cardiac condition as high doses of citalopram have been associated with cardiac rhythm disorders. There is probably no harmful cardiac effects in the low doses, however.

Avoid mixing them: I would not favor you taking supplements which have no proven medical value. Keep life simple as the drugs often have interactions which can complicate your life.

With caution: There is a caution about combining the two agents as both can increase the risk of bleeding, by different mechanisms. This however would be true of any serotonergic antidepressant. You should discuss any concerns you have the prescribing physicians.

Yes: A quick check of my drug interaction application shows no drug interactions for your 3 drugs. There are some concerns about proton pump inhibitors of stomach acid and plavix. Zantac and carafate (sucralfate) are not proton pump inhibitors.

Risk and benefits: Placement of stents in the brain vessels is tricky- we have to balance the tendency to form clot in the stent that can lead to strokes and the risk of bleeding from drugs like Plavix (clopidogrel) that we give you to counteract this tendency. Without knowing more about your specific situation I would recommend to discuss this with the doctor who performed the procedure and follow the instructions exactly!

Increase bleeding: Both of these medications affect your ability to clot by different pathways. While they do not interact in the body to create any toxic combination, they do have an ADDITIVE affect on thinning your blood and creating the potential of having a major bleeding problem. These are both prescribed medications and this SHOULD be discussed with your doctor.

No: Neither Effient nor Plavix cause shortness of breath. An evaluation for this, if not already done, should include: cbc, bnp, bmp, tsh, ddimer, cxr, echo, stress test, ekg, pfts and oximetry at a minimum. Ask your doc about these tests. The price of Effient 10 mg is $215 for 1 month. The price of clopidogrel (generic plavix) is $213 for 1 month. If you can find it for less, more power to you!

Effect on platelets: Has irreversible effect on platelets so must clear the drug from your system before you have operative procedure. Effect lasts longer than the half life of the medication in your blood.

Depends: It doesn't matter so much how many stents you have but why you have them, what do you your other arteries look like and when were they put in. For instance, if a patient had stents for a heart attack and got 2 stents and then got 2 more 8 months later and then another 2 in a year, I might recommend life long antiplatelet treatment with Aspirin (81 mg) and plavix (clopidogrel).

Not biologically: Plavix (clopidogrel) binds permanently to platelets -- with biological half life of 5-10 days -- this is what's important to remember.

Avoid NSAIDs/Aspirin: Nonsteroidal-antiinflammatory drugs (nsaids-such as motrin/aleve) and aspirin-based analgesic can interact with Plavix (clopidogrel) and increases risk of bleeding/stomach and intestinal ulcers/performation etc. Otherwise narcotic pain killers (some mixed with tylenol) are ok to take while being on plavix (clopidogrel). Like all medicine, use the smallest effective dose for shortest duration possible. Consult doc. Good luck.

It: It is unlikely that she will have any ill effects after taking an additional dose. You should contact her doctor to see if they want you to skip the dose for the next day. But I don't suspect that she will have any problem from that. If she had any bleeding or bruising, notify your doctor.
