Usually not: Ultrasound is very accurate about retained products of conception. You don't say how thick then endometrium is and that may be the reason for hysteroscopy and d&c ...Read more

Probably medication: If you are using vaginal Progesterone cream and now see clear fluid coming out of the vagina then most likely you are seeing the dissolved vaginal cream. Schedule a visit though to make sure nothing else is going on. ...Read more
