Possible MCTD: The highly positive ANA in a speckled pattern suggests mixed connective tissue disease. Mctd is a close cousin of lupus but usually has a better prognosis. Probably normal: Many people have a positive ANA without being sick. If you also have elevated liver enzymes, a workup for autoimmune hepatitis may be continued, but if there's nothing to suggest a liver problem biochemically, I'd not be in a hurry to diagnose despite anti-smooth-muscle. Possibly: Ps used to be derived from cow brains, so there was concern about transmitting infections. Now it is usually derived from plants. It has been studied and found to be safe up to 300 mg a day. Over that dose it may actually cause insomnia. It has not been studied for it's efficacy for insomnia. However, I have recommended it for patients who are very stressed and anxious and have had some success. ...Read more
