You should protect from the sun and use fading agents, hydroquinone being the best. You can get significant improvement with treatment but a cure remains elusive.

See a dermatologist: Details matter. You fail to specify any detail about the "medicine". Prescription creams are more effective than laser for melasma. Typical approach is to use sunscreen every AM. At night she would apply a cream combining a retinoic acid, a low strength steroid, a hydroquinone, maybe others. Expect to do this for several months. It is less costly and more effective than laser.
