DUB: This is abnormal bleeding and sounds like it is due to a minor hormone imbalance. You need to be checked for other causes including pregnancy problems if that is a possibility. Usually your cycle can get back on track with a simple medical treatment such as birth control pills or a short term hormonal med. You need a gyn exam and possibly some other tests such as bloodwork and ultrasound.

I have a Mirena (levonorgestrel) IUD, I've had it for a year now. And I'm starting to notice that my periods are lasting two week or longer. What could be wrong?

Change in pattern: Most patients with Mirena (levonorgestrel) have no periods or light periods. When patients report a sudden change in their bleeding patterns on Mirena (levonorgestrel) I want to see them in the office. I want to do an exam and sometimes a sonogram to confirm the IUD is still in correct location. Sometimes the IUD is partially expelled. Infections such as Gonorrhea and chlamydia can also cause bleeding and testing should be done

What is the reason for prolonged periods lasting more than 2 weeks?

Choice is up to you: Abnormal periods need evaluation; sonogram, lab tests. Non-surgical options include: birth control pills. Lysteda (non-hormonal rx taken during period), Mirena (levonorgestrel) iud. Minimally invasive options: endometrial ablation (in-office), hysteroscopy polyp removal or fibroid resection; and only if these treatments fail then consider hysterectomy - vaginal or laparoscopic if possible.

I am 44 years old and periods have been lasting about two weeks every time, and only have about two weeks between them. Is that normal?

Maybe, maybe not: Changes in periods deserve attention. 44 is younger than average age for menopause (51), but perimenopause can cause this kind of change. However, so can more serious things. Thyroid disease, diabetes, bleeding disorders, medications can do this, as can pregnancy! changes in the lining of the uterus including fibroids, "precancers" or cancer can cause bleeding too. Best to be seen.

Since I got off of birth control I've had two periods, each lasting for over 2 weeks with a thick, dark, stringy, yet very lite flow. Why is this?

Common problem: Irregular periods are one of the most common problems obgyns treat. If this is one time problem it usually resolves on its own. If this is a recurring problem you should be evaluated. Infections ; hormonal changes can cause spotting. If there is a concern for pregnancy then do a home pregnancy test. There are many options available to help regulate the menstrual cycle. Schedule a visit.

Could be normal: Depending on where you are in your reproductive life and what's going on (e.g. Stress, weight changes, diet changes, etc...), the regularity of your cycle can vary.

Options: An occasional off month is common and does not usually indicate a problem. If it continues see your obgyn for an exam and testing. Depending on your age and fertility plans there are lots of treatment options. Birth control pills can regulate your cycle. Mirena (levonorgestrel) iuds can minimize your periods. Lysteda is a great new hormone free drug used to stop heavy periods.

Nexplanon: You should have been counseled that the major side effect of progesterone only contraception is abnormal bleeding. This could be bleeding every day or no bleeding at all or anything in between. The bleeding pattern you describe, especially with early use, could be entirely normal. Keep a menstrual calendar to follow your pattern as one is developed. You may indeed have more bleeding than clear days

Light periods: The main reason for light periods is a change in hormone levels. Causes for what seems to be a light period can include: implantation bleeding in early pregnancy, thyroid problems, eating disorders, excessive exercise, initial periods for a girl, use of an intrauterine device, oral contraceptives & perimenopause.

See your doctor: These are most likely anovulatory periods and has overgrowth of endometrial tissue. Need to see your gyn for evaluation and maybe endometrial bx or D and C.
