Dysmenorrhea: Endometriosis and fibroids are two of the most common causes of dysmenorrhea (painful periods). Other common causes are ovarian cysts, varicose veins in the pelvis or something similar to endometriosis in the muscle of the uterus, called adenomyosis. Endometriosis?: See you Family Doc and get checked out for endometriosis. Your symptoms sound severe, especially to have started in your teens! It is especially telling if you have deep-penetration sex pain. Plan on having a pelvic exam and an ultrasound for a complete evaluation. If it's not endometriosis, then those investigations will take you closer to a diagnosis and then a treatment.

Maybe: Since it only happens during your period, it's possible it could be related to endometriosis. You need to talk to your MD.

STD?: The pea sized nodule you are feeling may be a lymph node and it may be enlarged due to an infection. The pelvic pain and back pain may be due to cervix infection. Usually these things occur together in something like Chlamydia infection and/or gonorrhea. Typically acquired sexually due to unprotected intercourse with a new partner or a partner who has acquired the infection from somewhere.

Yes: A pelvic exam to check the uterus and ovaries can be done at any time, including during heavy bleeding. If the pelvic exam is being done to check for cervical cancer or pre-cancer with a pap smear, it is best to do it when bleeding is minimal.
