My 3 year old is on her 2nd set of pe tubes (t&a removed).Left tube is in the ear canal & is infectioned.Is it recommended/safe to replace this tube?

Ear tubes: If the tube has come out and infections return, it is reasonable to replace the tube. Consider adenoidectomy if it hasn't been done yet. Don't worry, your child will most likely grow out of this. ...Read more

Highly unlikely: Making an incision and placing a tube in the ear drum can cause scarring. However, it is highly unlikely that the scarring would cause any hearing loss, even after repeated tube placement. Just let them know: RAD is not a reason not to have tubes, I have even used general anesthesia to stop a status asthmaticus in a kid with bad asthma (worked well when nothing else did). As long as the anesthesiologist knows the kids past history they can modify any plans they need to. ...Read more

ENT follow-up visit: During routine follow-up appointments, the ENT doctor will evaluate the child's pe tubes (position, obstruction, signs of infection, etc...). At that time, the doctor can remove earwax (cerumen) from the ear canal. Avoid using q-tips or other foreign objects to remove earwax (unless the wax is already at the outside opening), because one can unintentionally push earwax deeper into the canal. ...Read more

Very unusual: A middle ear fungal infection in an immunocompetent person would be highly unlikely. Can have fungal ear canal infections or fungi in secretions associated with pe tubes. If the infection doesn't respond to antimicrobial treatment based on culture of the drainage consider removal of tubes, adenoidectomy, antireflux treatment, blood test for antibody levels and lymphocyte enumeration, . ...Read more

May not be infected: It is not unusual for ear wax to appear brown or green. Some people have mostly wet and others dry wax. I presume that your eardrum has helped since then and even if it has not, I would expect to see some discomfort in that ear if it were infected. ...Read more

External ear infection: Is most probably your case, PE tubes site closes/heals on its own days after tubes are out. Need to see your doctor/ENT for the ear discharge you currently have. Topical Lidocaine: Do not worry about it being expired. Do not use Lidocaine for PE. Give it a couple of days to wear off and you should be back to normal, if not, then see your PCP. ...Read more

Phenylepherine: is a potent systemic decongestant, and is sold OTC, you want to be cautious using it long term on your own, please read the labels carefully for side effects and drug-drug interaction. Saline nasal washes, and at times steroid nasal sprays can help too, but that doesn't replace doctor's evaluation.
