Go see a doctor: You need to find the cause. This will require a visit to a doctor to check you and find out who you have not had a bm and what reason for your urine problem. Valsalva maneuver: Straining at defecation causes an abrupt drop in bp. Although you note that your BP is "normal", it's the fall in BP that causes dizziness and it's likely that yours falls with valsalva (straining) as does everyone's. You normally should not be straining or pushing during bowel movements. If you are having pain as well, you may have an anal fissure. This is a crack in the anal opening. Can be painful and also make anus feel very tight.

Hemroids: This is the most obvious answer but not the only correct answer. Hemroids are enlarged blood vessels that can be fragile and bleed easily and also painful around the anus both externally and internally. But you could have a colon problem and the blood could be coming from there. Pain vs no pain? : Bleeding with bm and if associated with much pain, likely anal fissure and/or external hemorrhoids. Internal hemorrhoid usually does not hurt and blood is often found coating the stool rather than mixing with it. Better safe than sorry, check with your doc to see what you have. Sometimes but...: Straining at stool on occasion may promote colonic emptying , but you do so at a cost--increased risk of hemorrhoids, anal fissures, & even passing out. Unclear, but...: At "ejaculating" during MY, have you experienced the sense of orgasm or just saw few drops of semen-like fluid dripping out along with BM especially at straining to defecate well-formed stool? It depends :
If blood in the stoll is red, it may be from a hemorrhoid. This needs to be evaluated and treated, but is fairly common.
If the blood or stool is darker (black) and has a 'tar-like' appearance, it is more indicative of a bleed in the upper areas of the GI tract. This is more significant and needs more prompt evaluation and treatment.

5 weeks after episiotomy i think I may have torn while trying to pass a form bowel movement. Is this possible?
Not likely: Episiotomy tends to heal very quickly because of the rich blood supply to that tissue. Although it is possible to tear the episiotomy open by passing a large hard bowel movement, it is unlikely that this would occur after 5 weeks. Nausea can be due to many different causes and I see you are taking Advil (ibuprofen).Motrin and Acetaminophen.Motrin and Advil (ibuprofen) are same drug IBUPROPHEN and it can cause nausea/vomiting and other GI Symptoms
