They really can't be adequately assessed through an email format. It sounds like it is not exclusively occurring while asleep, when falling asleep or when waking from sleep. (?) I suggest a comprehesive neurological evaluation if one has not already been done. If symptoms occur just prior to falling asleep or as you are waking up, a polysomnogram (sleep study) would be a valuable diagnostic tool.
