Trigger point?: Could be a lymph node, an area of muscle fibers that contracted and formed a small mass (overuse or after trauma) and/or may represent a "knot" or trigger point that can respond to tissue massage. If not growing or and painless ok to observe;get it checked out if growing/painful or persists (to make sure it's not something serious like a malignant growth)

I have swollen lymph nodes in my neck and a lump on my upper back. All are painless. Should I see a doctor about this?

Yes: Most lumps in back are sebaceous cysts or lipomas. If a lesion there were malignant it would drain to axilla and not neck. The lump should be examined to define its nature and then if excsion not indicated, the nature of the cervical nodes should be evaluated. This can only be accomplished by a physician.

Needs to be checked: I suggest you have it checked by your family doctor. It may be an infection like an abscess or a tumor causin swelling and pain and it is getting worse and you are not able to sit. If you cant get an appointment with your doctor then go to ER

Evaluate and remove: It sounds like a subtype of a lipoma with more fibrous tissue that tends to be more symptomatic for people and occurs in the upper back area. I would recommend seeing a dermatologist or a surgeon for evaluation ant remove it most likely within an hour under local anesthesia. They may want to image it to ensure it isn't impinging on vital nerves.

See a dermatologist: It is difficult to make a proper diagnosis since I am not there to see the condition. I recommend seeing a dermatologist so that they can make a proper diagnosis and give you something to get rid of the condition or relieve symptoms.

Hard to say: Without being able to see the area, but one possibility is shingles. It sounds like its very painful for you. Please consider seeing your doctor as there are helpful treatments available.

Cyst: It is possible that you have a sebaceous cyst with infection. There are other possibilities of course but it cannot be accurately determined without an examination. You should see your doctor for evaluation.

Blocked gland?: A sebaceous cyst is a collection of oily fluid that occurs when an oil gland is blocked. Some folks are more prone to them than other and they often appear on the neck, shoulders, and back. Try to avoid skin products that are oily. Stay away from transfats and bad oils, which is good for overall health. See doc for proper diagnosis because it could be more serious.

Hard mass with pain: Please see your primary care physician for exam and imaging if indicated. Severe pain is not to be ignored. I am sure you have been worried about things like cancer. Best not to put off the exam so you can relieve anxiety and get help. If the pain is unbearable, urgent care or ER for treatment.

2 options: Two things come to mind. The most likely is small cysts that are becoming irritated or infected. The other possibility is lymph nodes that are swelling up. Either way you should get it checked because either condition might require treatment. I would say it's not terribly concerning but if left without treatment could get worse.

Lymph node?: It sounds like an enlarged lymph node. The soft tender spot is probably what the lymph node is associated with. If it gets worse instead of better, see a doctor. I wish you the best!
