Tendonitis: If you're sure it's not a splinter it could be irritation of the fibrous connections under your toe. In my experience gout does often occur at the big joint of the big toe, but it is more severe than splinter pain. Take Aspirin or Acetaminophen (generic tylenol) and watch what happens. Few things: Could cause this. First, I would examine the skin to see if there is any sign of cracked skin, blistering or scaling as a fissure or athletes foot may be present. If not, I would question you with regards to low back pain or if you are possibly diabetic. Keep in mind it takes an average of nine months for the nail to grow from the base to the tip so it will take a long time. In extraordinarily rare circumstances it could be skin cancer.
