See PMR doc: You are a female in late 60s, and the tendency is for head forward shoulder forward positions. This pinches the cervical outflow, and pulls the shoulder blades up and out, resulting in soft tissue pain. More Info: The "hump" is associated with Cushing's syndrome however there are a lot of other symptoms/signs You should see your Primary Care Provider since it appears you are worried about this possibility. There is EFFECTIVE TREATMENT for Cushing's so see your Doctor soon!

Commonest cause of--: Such backache is generally due to tiredness, muscle pull/strain, from regular day to day activities, either too much of a normal activity or some unusual one, which can happen to anyone.
Pain meds like a nsaid, applying heat/ice packs and regular stretching exercises woud all help.

Not fun!: That sounds like a muscle spasm and possible pinched nerve - pretty commonly seen in the urgent care where I work.
