Iliotibial band: could be inflammation of distal iliotibial band, could be knee arthritis or meniscal tear. I would start with NSAID treatment, barring no contraindications (ulcer, kidney disease). I would at least obtain an X ray of the knee and trial of physical therapy. You can seek interventional pain medicine, and seek injections.
