I like a firm diagnosis. If this is compromise of a lower lumbar nerve root, let's see it on an MRI. This may direct treatment/help with prognosis.
Under 50 I would expect a disc problem but arthritis of a joint in the back may also contribute. Physical exam can also help show disc pathology but is not as sensitive/specific as MRI.

2nd opinion: If you have not had a Doppler ultrasound to rule out a clot, you may want to go for second opinion. The exam for a blood clot is only 50 percent sensitive, meaning that exam findings can be misleading.
