Many possibilities: Pain from the shoulder down the arm my be caused by a variety of things. Starting at the neck, there may be a pinched nerve, or the joints in the neck may be irritated. The muscles around the neck and shoulder may develop trigger points or tendonopathy which cause radiating pain. The shoulder itself may develop impingement. More likely muscle: Typically the sound is muscle sliding over a joint. It can pinch a nerve but more often catches tendon part. Need exam: Your symptoms suggest cubital tunnel syndrome ( ulnar nerve entrapment @ elbow) but a thorough history and physical exam is required to confirm. Supraspinatus( rotator cuff) injuries can occasionally present as you describe but this would be unusual. Pain there can be from injury to the muscle, impingement to its tendon (the rotator cuff) or from irritation to the nerve that goes to the muscle. Two considerations: You might possess pinching of nerve in neck, cervical radiculopathy, and if arm is raised, you might feel less symptoms. Sounds like: Nerve irritation or injury that could be coming from your neck seem a very likely explanation for these symptoms. Assessing if neck position change the symptoms might help. A good neurological history and competent examination should sort this out.
