Yes if unstable: The neck and shoulder blade pain can be a sign of lower cervical disk disease. The pain you are describing may be a combination of more than one problem. A torn labrum does not always require surgery. If the shoulder is unstable then you will require surgery to control the pain and prevent further shoulder joint damage. Sounds like a nerve: A pinched nerve may give you the symptoms you are experiencing. Other things can cause pain like this may come from reffered pain . Sometimes gallbladder problems can radiate to the right shoulder. A thorough history and physical will point your doctor in the right direction. A cat scan of the cervical spine and rt shoulder may help. An emg/nct study of the rt upper extremity may be required. ...Read more

I've had mild to extreme shoulder blade/neck/shoulder pain for four years, pain increased with caffeine beverages, what could this be?

Possible Migraine: It's possible that this may be a migraine variant. It's possible to see pain in the neck and shoulder area with some folks. Another possibility is a pinched nerve but that wouldn't explain the caffeine connection. Our bodies never read the textbooks, so anything is possible. Have it checked out. ...Read more

Muscular Pain?: Hi, your recent onset of symptoms and your age suggest that at this point the most likely diagnosis is that you have a shoulder/upper back/neck strain. You don't describe radicular symptoms that would suggest a neurologic etiology. Initial treatment after an H&P and imaging usually starts with NSAIDs and maybe a muscle relaxer and also some therapy exercises to see if your symptoms will get better ...Read more
