Depends on history: If you had a trauma (fall, car accident, etc) then a fractured bone is possible and you should get an xray or ct scan asap. If no trauma, you could still have a pinched nerve and evaluation with health care provider and imaging is still a good idea.

Referred pain: When a person has pain along the side/neck/shoulder related to an abdominal problem we usually think the diaphragm is involved. The diaphragm is the muscle which aids breathing (inspiration/expiration). Abdominal inflammation or partial bowel obstruction causes pressure on the diaphragm which is referred to the shoulder. It should resolve as your abdominal symptoms improve. Rx OTC pain killers:)

Evaluate Neck: If you are not improving have your doctor order a cervical spine mri. Many times a herniated disc in the neck can cause arm pain especially if you have not improved with the medications and physical therapy. If it is a herniated disc you may need evaluation from a spine specialist to determine what other options might be needed (injections, surgery, etc) to help reduce pain.

Radiculopathy: Neck pain that radiates down the arm is known as radiculopathy. Similarly, sciatica, for example, is due to pressure on one or several of the nerves exiting the lower spine, usually from disc material or bone growth. This can occur in the neck as well. In common terms...A pinched nerve.

Pinched nerve: The nerves to the arm come out from the spinal cord from the neck vertebrae. A pinched nerve, at this level, could cause these symptoms. See your doctor if this persists, epically if there is numbness and/or weakness.

Cervical nodes: Yes, you need to be concerned and have them evaluated by specialist-----there are benign reason for the enlargements but there is concerns for cancer----do not wait have them evaluated

Sounds: Like enlarged lymph node. Show it to your doctor who prescribed antibiotics for you. It might be caused by an organism that is resistant to it. Or it could be caused by something else.

Nerve root injury: Your description is suggestive of someone that is suffering from cervical radiculitis. Injury to the nerve root as it exits the foramen can lead to pain, pins and needles (paresthesias), numbness, and/or weakness. In someone of your age group the most likely cause is a disc herniation. I would suggest that you meet with your doctor to discuss further treatment.

See your doctor: It's probably best to see a doctor. Your symptoms could be as simple as muscle strain/spasm, but could also represent compression of a nerve by a disc in your neck, or compression of a nerve as it comes through muscles in your neck. Shoulder pain and issues like rotator cuff problems can also radiate up into your neck. In the mean-time, acetaminophen, ibuprofen (or Aleve) and apply heat.

Yes: Torn ligaments in the shoulder are painful. See your doctor if the pain persists and if you are having trouble moving your arm like reaching overhead or behind your back.

See a physician: This could be a large number of things, some serious, such as an upper extremity blood clot and even a heart attack can cause some these issues. Be safe and get checked out. Also make sure you have regular breast exams as breast cancer that has reached the lymph system on the left side can cause this. After you clear these possibilities, look at thoracic outlet and muscular issues.

Be rechecked: If you are experiencing severe pain on the left side of your neck down into your arm, it is probably more than a simple injury to your "bursa". Among possible causes which could be checked for would typically be a pinched nerve in the neck, a torn rotator cuff, or signs of cardiac ischemia. A detailed history, exam, and appropriate studies would likely result in an accurate diagnosis.

Could be: That could be serious. The most troublesome thing you said is that you have chest pain in the center. Undiagnosed chest pain in a 47 year old who reports, as you do, a history of angina, should be further evaluated, especially if it's new, occurring more often, with less effort, lasting longer, or getting worse.

May be heart related: Yes! The location of radiating pain is not specific, and it can be cardiac (heart) related whenever there is chest pain - radiating or not. Whether it goes into your tongue is irrelevant - it's possibly a very serious indication that a heart attack is imminent. It should always be ruled out, so get an exam for this very soon.

Aching upper body: Would check for strep throat, even if not symptomatic, or mycoplasma infection may also be TMJ or bruxism (grinding teeth at night), reflux check for h pylori as well, check ESR, CBC, CPK, ANA for autoimmune process, though less likely check EKG, troponin, check CXR and for lymph nodes / thyroid would follow up with doc ER today.

Pinched nerve: A pinched nerve in the neck can cause a tingling or shooting pain sensation that might radiate in the neck and down an arm. The problem could be related to a disk issue in your neck. An MRI could possibly look into this further. I hope this helps.
