In the first few weeks after major open abdominal surgery, there sometimes can be some back muscle pains due to abnormal posture while sitting or standing. May be: Something else. How long ago was your surgery? If the surgery was recent then muscle and gas pains can come and go & sometimes can be worse with large or fatty meals. These are usually managed by the placement of drains: endoscopic (into the bile duct) +/- percutaneous (into the fluid collection). Check with your doc: If this is newly developing, there sometimes may be fluid collection around the liver after the surgery. There are different things that can lead to this, so difficult to tell you simply hear. But the sensory nerves for that part of the body refer pain to the right shoulder — so the right shoulder pain has to do with the area around the liver.

As for the R-sided rib pain in the area of the surgery, it is not uncommon to have some residual soreness from the procedure itself. Rare condition is pain from retained stone in the bile duct. May need blood tests & new ultrasound.
