Back vs hip: In an older person we worry about hip arthritis causing the pain in the hip and back. But in someone your age, the primary location is more likely the back, with muscle spasms mechanically affecting the hip. A course of physical therapy focused on the low back would likely help. If it hurts when: You are walking the most likely thing in the absence of fever would be sacroiliitis, inflammation of the sacroiliac joint. If you were running a fever and there is tenderness where the last rib meets your spine, then i would be concerned about a kidney infection. If you are walking and you feel like the pain is shooting down the back of your leg then i would suspect you were suffering from sciatica. Most likely however you have pulled a muscle.
