Have you tried topical OTC meds (over the counter) like biofreeze, aspercreme,or bengay? You might have some irritation to your thoracic facet joints. If topical OTCs are not effective, may benefit from Voltaren (diclofenac) gel (dull, ache, throb pain) or lidocaine gel (stab, burn pins&needles pain). PT may help. ...Read more

Upper back and L sid: There are many potential causes: herniated cervical or thoracic discs vs localized muscle strain vs facet joint irritation vs referred pain in particular from abdomen/diaphram which refers pain into shoulder/upper back. Pulsations: Those pulsations if synchronous with your heart beat or pulse may be the aorta which is the largest artery in your body and runs through your torso and up against the spine.
