In the first few weeks after major open abdominal surgery, there sometimes can be some back muscle pains due to abnormal posture while sitting or standing. Check with your doc: If this is newly developing, there sometimes may be fluid collection around the liver after the surgery. There are different things that can lead to this, so difficult to tell you simply hear. But the sensory nerves for that part of the body refer pain to the right shoulder - so the right shoulder pain has to do with the area around the liver.

Rare condition is pain from retained stone in the bile duct. May need blood tests & new ultrasound.
