Pleasant sensation: Deep to pubic bone rather than in the groin. You could probably ejaculate a second time after about 30 minutes if you can get excited enough. Good Luck

Could be: Hernias are usually associated with a lump, but in the early stages may not be. See your primary care or a general surgeon for a physical to check it out if it persists or gets worse.

Urologist can check: A primary care doctor or a urologist can check to see why there is pain in the shaft of the penis, and also look into the groin pain and the lack of ejaculation. An alpha-blocker such as Flomax (tamsulosin) can cause retrograde ejaculation (goes into the bladder). Pain in the shaft sounds like it needs to be taken care of, so a urologist should take a look.

Urologist: Have you been examined by a urologist? Your symptoms suggest possible epididymitis which may not show up on ultrasound. Another possibility is a small inguinal hernia which can be diagnosed by physical examination by the urologist, your physician or a general surgeon.

Many causes: Your pain could represent many things. Your vas deferens transports sperm from your testicles via your groin. You may have a hernia if there is a bulge present. Prostatitis is usually accompanied by fever, pelvic/rectal pain or pain when you urinate. A burning sensation on your thigh is usually a result of a nerve problem. I would recommend a physical exam to assess your problem.

STD unlikely: No STD is a likely cause of such symptoms. However, if you consider yourself at risk for STDs, it would be a good idea to be tested. But not because of these particular symptoms.

Nonspecific: Unfortunately your symptoms are fairly nonspecific. Potential causes range from upper lumbar disc herniation to groin pull to hip pathology (i.e. Cartilage injury, stress fracture). I would suggest anti-inflammatory meds and backing off on the running for some time. If symptoms do not improve then definitely see a physician.

Number of causes: Groin pain and painful ejaculations have a number of causes. Some are: urethritis (the canal inside the penis), prostatitis (inflammation or infection of prostate gland, orchitis (inflammation of the testicle), epididymitis (gland where semen produced). Definitely should see a urologist.

May be prostatitis: Abdominal groin the thigh pain going into testicle may also be prostatitis. If it doesn't go away with rest and antiinflammatories go to a urologist to be checked.

Rest: Ibuprofen will help with pain and inflammation. I suggest a "sexual vacation": no masturbation for 3 days (sure...you can!)- If related to unprotected sexual intercourse- get examined and tested for STDs. Force fluids (8-10 glasses of water daily).

Scrotal Pain: First and foremost given this scenario is an inguinal hernia. Second is a spermatocele or varicocele in the scrotum. And, third is a pinched nerve in your back at l4-l5 that is causing a pain inferior to the nerve injury.

No: These symptoms can be due to chronic inflammation of the testicles or epididymis or prostatitis. A thorough history snd exam should be able to point to the possible cause. Do see your family physician or a urologist got further advice

Unlikely from nerve: No reason why your urethral meatus should be sore after ejaculation as result of trapped nerve. You had a cystoscopy which would have ruled out meatal stenosis or urethral srticture. Symptoms could be from prostatitis which might be abacterial if symptoms failed to improve on Cipro (ciprofloxacin). Have you seen a urologist? Or considered second urologic opinion if you have already seen one?
