I have left chest pain that radiates to my back with worse pain with deep breaths. Have elevated d dimer but negative CT scan for PE.

Multiple things: Hard to tell without a physical exam, but you could have caused some muscle spasms leading to this feeling, jarred your costocondral or costovertebral joints, subluxed a rib/ribs, jammed your shoulder causing pain to entire shoulder girdle, or irritated a nerve in your neck or back with radicular signs. Given your age, gender and level of fitness, the most likely cause is "musculoskeletal", that is related to the tendons, bones and muscles in your body; rather than your heart itself.
