Get evaluated: Sometimes shoulder pain can overlap with cervical spine radiculopathy and therefore will not respond to injection. In addition to this you could have an underlying injury to your rotator cuff which is not responding to injection. Either way an MRI of the shoulder might be indicated if your pain has not resolved. If this is negative then there might be a neurological component that needs a workup. ...Read more

Tendon or bursitis: I suspect during your workout at the gym you pulled muscles or tendons and developing tendinitis or bursitis or both in your shoulder. It would be best to ice the shoulder especially for the first twdays after the injury for the next day or so and then use heat after that. And maintain range of motion.
