I have l5s1 disc herniation pain left buttock leg calf and foot , difficulty walking and very painful sitting pain is now radiating to left testicle ?

Nerve irritation: Buttock pain that radiates down a leg most likely is the result of a nerve irritation. For most people the : Leg pain with or without buttock pain is the main symptom. There is usually no or minimal back pain although it may start with back pain and then as the leg pain starts or worsens , the back pain decreases or resolves. The pain is due to direct nerve compression. The timing of your pain, however, is not typical as most feel better at rest . Exam, studies, injec: It can be hard and piriformis syndrome as a diagnosis is likely often missed. Physical exam can help, a nerve conduction with the hip in neutral verses internal rotation can tell, response to injection into piriformis is most diagnostic. Need removal: Typically a CA-125 level would be checked to help determine the likelihood of malignancy. Sounds low based on the US findings. Regardless, the mass should be removed to relieve your symptoms and so that a pathologist can evaluate the ovary. Most times both tubes and ovaries would be removed and sometimes the uterus. Delaying in this: Circumstance isn't advisable. Your symptoms suggest impingement, either through direct nerve compression or swelling around the nerve. In any event, a discussion with your neurologist/neurosurgeon about an epidural steroid (or transforaminal steroid) injection should happen sooner, rather than later.
