Need evaluation: If the doctor seeing you doesn't know, there is little a consult on the internet can do. You might want a second opinion. This could be herpes of the geniculate ganglion, a form of shingles, low grade dental infection spreading to the fascial planes of your neck and so on and so on. You might see an ENT specialist. Ear and neck: Please start by seeing an Ear specialist!
If the ear is the only issue it will be taken care of.
If the side of the neck is troubling to the doctor you will be referred to a neck specialist.

Possibilities: You may well have disc problem in your neck compressing nerve going down left arm, but thoracic outlet syndrome could present this way. Unlikely at your age that this involves heart but would not hurt to have your doctor do an EKG. Less likely is a jaw issue, TMJ, but if evaluation is non-diagnostic, check in with your dentist. ...Read more

What can cause pain behind right ear that radiates down neck into arm? Pain increases with inhaling and stops when holding breath. No cold symptoms.

Radiculopathy: Pain that radiates down the arm can reflect a cervical radiculopathy (shooting pains due to a pinched nerve in the cervical spine). I'm not sure what to make of the association with breathing, although conceivably a pancoast tumor (a tumor a the top of the lungs) could cause the constellation of symptoms you describe. A tumor would be very rare, but see your physician if symptoms persist. ...Read more

Not typical of TMJ: Your description is not typical of dental mediated problems. That said, have your dentist check you to rule out clenching, grinding, malocclusion, abscess, gum infection. Need an exam: It's possible you have an infection in the ear canal or behind the ear drum. However your symptoms are somewhat atypical for these causes. It's possible the source of your pain is somewhere else in the in the pharynx and is being "referred" to the ear area. You need to see your physician as soon as possible. If the cause isn't apparent you need to see an ENT specialist.
