And is likely an incidental finding on ultrasound. The fibroid you described as well is also benign and small. The most important and definitive opinion comes from your doctor who knows you the best. Ovarian cyst: The ovarian cyst with septations is small, and probably benign. The myometrial fibroid is also probably benign. A trial of "ovral" (a birth control pill) may diminish both in time, but your doctor may also want a sample of tissue from the ovarian cyst which is gotten via laparoscopy because of the septations. Fibroids : Fibroids are very common and they can cause many symptoms such as abnormal bleeding, pain, pelvic pressure, and infertility or miscarriage. Fibroids are also very controversial in how much they contribute to these conditions as women with fibroids can have no symptoms at all. Fibroids usually shrink after menopause when your reproductive hormone levels fall. Ask 10 doctors what to do with fibroids and you will get several different answers. We all probably agree that when the symptoms are significant and we cannot find another cause then surgery is helpful. We sometimes remove just the fibroids (myomectomy), shrink or stabilize them them medically (bcp, gnrh agonists, progesterone) or radical surgery (hysterectomy). Sometime size alone is a reason to operate. If you are younger and the size is larger and getting larger, then myomectomy may be indicated just for size. We also worry more about possible cancer if the growth rate is very rapid. The decision regarding which therapy has to do with your life goals and medical condition. If yo want more children then hysterectomy is obviously not a good option. Medical therapy sometime works but some of the medications have side effects. A degenerative fibroid means that the tissue has died and it is regressing. Degenerative fibroids can be very painful. Your does not seems to be causing symptoms. You need to work with your physician to understand why he/she suggested the pill. If you are worried about the side effects of the medication then you may be better served with another approach. Liquid vs other stuf: A simple cyst generally contains only fluid. A complex cyst is a sac that contains mostly fluid (liquid) but also some material - cells / tissue or colloid or something solid - inside. That solid material makes its appearance different than just a fluid-filled sac. Gyn consult: Certainly an adnexal mass is of concern. Often a careful evaluation by ultrasound can differentiate benign from malignant masses. Breakthrough bleed: Sounds like breakthrough bleeding which occurs when you have bleeding at unscheduled times while on the pill. Often things get back to normal the next cycle. Possibly, only: The low amh and low afc makes us concerned about your fertility potential so complete family asap. No afc does not necessarily mean the ovary is completely useless. The ovary makes hormone even if there are few eggs. The big picture for you is that your fertility is on the down side and you need to get advice from a futility expert to make good decisions. Probably ok: It depends on the surgeon's training and experience. Ask how many cases like yours he/she has done. While volume alone doesn't equal quality, it's a reasonable surrogate. At least 10-20 laparoscopic ovarian procedures per month would be ok. Also ask about their comfort with the size and complexity, and if taking precaution to avoid breaking and spilling cyst contents.
