See your provider: This sounds like a normal size uterus and normal size ovary. Endometriomas are ovarian cysts which contain old blood in them. 18 mm is pretty small. If you haven't had it, you might still benefit from a laparoscopy to look for and treat endometriosis especially if you have 4 years of infertility. Time: Clomid (clomiphene) doesnt work every time. In some women, it has an opposite effect. We like to see at least 16mm follicle for a mature egg. Your doctor may make more adjustments to the dose and days you take it to see if you get a better response. There are other meds like Femara that also induce ovulation. Limited Information: Almost impossible to help you with this information. There are so many different scenarios this represents. The most likely is a left over follicle from the last cycle that didn't physically ovulate (lufs syndrome). This is not indicative of any serious problem.
