Go to your doctor. : Self diagnosis is very risky, especially with appendicitis. Go to your doctor or the er. A ct would be the best test to distinguish between a cyst and appendicitis. Get checked though because if it is appendicitis, delay in diagnosis can lead to rupture which can cause fertility problems in the future, or even death. ...Read more

I : I am very concerned about several things here. That is way too much Ibuprofen (advil and Motrin are other words for ibuprofen). Taking 800 mg every 6 hours for a few days is a lot, but taking 800 mg every two hours is very concerning, and can cause side effects with your stomach and your kidneys. You need to be immediately evaluated for this pain and irregular bleeding. It is not normal. Sometimes these symptoms can occur in the first few months after a mirena, (levonorgestrel) but you state you are 25 months out, so these symptoms are very abnormal. Please have a complete gynecological evaluation. You likely need an imaging study such as an ultrasound. There are other pain medications that you can be treated with. Pelvic abscess: The high WBC count worries me. Hemorrhagic cyst may be associated with an abscess. It may not have been infected before but is now infected (abscess is usually due to a focal bacterial infection). I suspect that your doctor put you on antibiotics. If you are getting worse on the antibiotics, they may not be working. Call your doctor right away to let the doctor know it is worsening. ...Read more

Please see a gyn: As you need followup , the er should have referred you to or consulted with the gyn on call as you may need a laparoscopy to see what is going on a ruptured cyst can cause in some cases an infection or peritoneal irritation or peritonitis so you need to see a gyn. ...Read more

Can complex fluid filled ovarian cysts become cancerous? Why might there be a bright ring around it on u/s then no ring on f/u u/s? Causing much pain.

Yes it can: Complex cysts can be cancerous depending on their size and rate of growth. A CA125 test can help decide its nature. Ultrasound is a good test to measure increase in size and also may show any solid components inside the cyst.
