A 27cm ovarian cyst has a low chance of being cancer in a young women in her 20s; however, in an women in her 60s it would be very worrisome for cancer. Now that it has been demosntrated to be elevated in the setting of ovarian cycsts further evaluation should take place. The likley next step would be pelvic imaging with an US to urther characterize the cysts.
