Oligoclonal Bands (Overview)

Oligoclonal bands are bands of immunoglobulins that are seen when a patient's blood serum, gained from blood plasma, or cerebrospinal fluid (CSF) is analyzed. Oligoclonal bands are an important indicator in the diagnosis of multiple sclerosis. Approximately 79%-90% of all patients with multiple sclerosis have permanently observable oligoclonal bands in their csf.

Multiple sclerosis: Presence of CSF oligoclonal bands is consistent and normally diagnostic of multiple sclerosis in the right circumstances. The absence does not rule out MS and the diagnosis has to be based on clinical findings and MRI results. Other causes: Oligoclonal bands can be present in some other conditions such as infections and inflammation of the central nervous system. Increasing the threshold for diagnosing MS by requiring at least 4 oligoclonal bands, allows to exclude these other causes and diagnose MS with greater certainty. ...Read more

Consistent: But diagnosis of MS should be based on history, physical exam, results of MRI films, and spinal fluid is useful in ambiguous cases. However, elevation of oligoclonal bands can be seen in several disorders and NOT definitive on its own. Have your doctor now put the whole picture in place. Cured? Strong word: Your syndrome unclear. Lhermitte's is a sign seen in spinal cord disease. CIS is often an abbreviation for carcinoma in situ. What "CIS" are you referring to? "Cured" a strong word -meaing illness CAUSE has resolved. Can you prove that? Bands seen in autoimmune processes - may produce intermittent syndromes. Good to hope. ...Read more

Suggest : The oligoclonal bands should be significantly increased in spinal fluid as contrasted to serum to be c/w multiple sclerosis, and some of your problems are likewise supportive of ms. However, do you have history of eye and skin problems more in keeping with behcets, and are MRI studies of both head and neck c/w ms? This should be fairly easy to sort out, and if ambiguous, perhaps a second opinion. ...Read more

CIS: Lhermitte's sign is consistent with spinal cord lesion, and bands are about 95% predictive of onset of clinically definite ms in future, but if no lesions seen on either mri, it is not fully clear that your future deflinitely involves ms. If no symptoms, recommend supplementing vitamin d-3, approx 5000 units twice daily (level 50-60 ng/ml), and repeating mri's in 6 months, and if spots, start med. ...Read more
